Obtained when the highest die you rolled (after cut) is a 1, 2 or 3. If this happens, you have failed at your goal¹, and usually will take Doubt and/or other penalties, and won’t gain Fame.

Disastrous Inspiration

If you were invoking Inspiration there’s an additional risk - if you would go into Turmoil go into Madness instead.
